Description

Provides accommodation services including respite, supported living and residential. Also provides day opportunities to adults with substantial and critical need as identified through adult social care assessment. Shared Lives is an adult placement scheme that includes respite care, permanent care and outreach for day opportunities is offered. The day service is available on Monday to Friday 9am until 5pm. Shared Lives is flexible and includes evenings and weekends.

Referral

Young people and adults are referred to the service following an adult social care assessment and agreement about services required.

Costs

Tariff is available. There is a charge for transport to day opportunities. Care cost for accommodation is subject to a means test.
