Description

The Physical Disability Team works with adults aged 19 – 75 who are living with a physical disability. This includes adults with an acquired brain injury, illness affecting physical functioning as well as support for disabled parents. The team assesses and reviews needs. Those who are eligible are offered a personal budget to support with meeting needs. This can be managed independently or the team can arrange services. These can include support at home, short stay (respite care) and residential care. There is currently a 6 - 8 week waiting list for an assessment. Informal carers are offered a carers assessment to identify any support available.

Eligibility

Following an assessment, level of need is measured against a criteria set out by the Care Act 2014.

Individuals can self refer, please see contact details above. Referrals are accepted from third parties or other professionals with consent of the individual.

Costs

Contacting the team for advice and information and completion of a care and support assessment or carers assessment is free of charge. Dependent on your financial situation, there may be a contribution towards the cost of a service; a financial assessment is completed to identify this.
