Description

The Learning Disability Partnership Young Adults Team (LPDYA) is a countywide team, which supports people with a learning disability to lead as full and independent a life as possible. The team is part of the wider Learning Disability Partnership that brings together social care and health professionals in integrated teams.
The LDPYA team:

  • Complete social care and specialist health assessments and reviews

  • Develop support plans to meet a person’s assessed eligible needs that promotes independence and meets identified outcomes. Services could include Assistive Technology, Intensive Assessment Support Services, short breaks, supported living, support in the person's home, day time opportunities and residential and nursing home placements.
For young people with a learning disability who do not need social care before the age 18, and there is a likelihood that they may need support from adult social care, the team will start discussions and planning for adulthood at an appropriate point with a young person and their family/carer from the age of 16 as part of their Education, Health and Care Plan.

Eligibility

The LPDYA team supports people with diagnoses of a learning disability from the age of 18 until a time suitable to them as an individual when they will transfer to a long term Locality LDP Care Manager. This is likely to be (but not automatically) at the end of formal education.

To receive social care support any potential young person will need to meet the national eligibility criteria for social care.

Costs

An assessed contribution to care support in adulthood will be required.
