Description

The Children’s Continuing Care Team work with children and young people under 18 who need a tailored package of care because of their disability, an accident or illness. Children and their families are fully involved in the assessment process with their views and preferences central to the development of bespoke care packages to meet the assessed need. Care is delivered over a 24 hour period; access to information is available during office hours.
The team work closely with other services who may also be involved with a child or young person, for example, other health services, social care, education, and the voluntary sector.

Eligibility

A child or young person whose needs arising from disability, accident or illness cannot be met by existing universal or specialist services alone. This includes health, social and educational needs which may require support from a range of services over a period of time. Children and young people who are referred to the service will be screened for eligibility using an adapted Decision Support Tool. If the young person is likely to meet criteria then a full assessment will be undertaken. All children and young people should have joint assessments with Children’s Social Care and education.
The provision of a care package is agreed at Disabled Children’s Panel or County Resourcing Panel (both panels are made up of senior managers and Commissioners from Health, Social Care and Education services).

Referral

Children and young people can be referred to the team by their doctor, community nursing team, hospital or their social worker from Disabled Children's Social Care. Some children and young people may be referred by a voluntary or community organisation such as East Anglia's Children's Hospices (EACH). Children and young people who are already known to the team can self refer. The team works across Cambridgeshire, but does not cover Peterborough or Fenland; children and young people in Peterborough or Fenland will be referred to their relevant team.
