Description

At the centre a range of activities and opportunities are provided that provide clear outcomes for individuals. Four different types of activities may be offered:

  • Day break: Supporting people with complex physical and sensory needs to be part of their local community and have their individual needs meet by staff who are highly skilled at working in this area.

  • Live to learn: For people who are on the Autism Spectrum who may need very clear visual and environmental communication support. Opportunities from small group activity to specialist 1-1 support services may be provided.

  • Out and about: This provides opportunities for getting people skilled up for living independently; it involves accessing the community more of the time rather than being building based.

  • Train to achieve and gain: Vocational qualified staff work with individuals helping them to gain skills, experience and qualifications via the Open College Network, to show work readiness or offer places in a supported work environment.

People with eligible needs as identified through an adult social care assessment.

Referral

Young people and adults are referred to the service through the social care team following an adult social care assessment and agreement about the services required. Self referrals can also be made for self-funding arrangements under certain circumstances.
