Description

The Cambridgeshire Learning Disability Partnership (LDP) includes specialist social care and community health professionals who help people in Cambridgeshire who are over 18 years of age who have a learning disability to lead full and rewarding lives.

Eligibility

To receive health support people need to meet the diagnostic criteria of a learning disability.

To receive social care support people additionally need to meet Care Act eligibility criteria.

Referral

Self referral or by a carer, relative or friend. Via your GP or other health professionals or by people who are paid to support you.

Costs

An assessed contribution to care support may be required.
